Eviction Prevention Program Guidelines and Documentation


Household income eligibility is based upon the 80% and below the area medium income for the household in the county they reside.


  • Please have these documents available for review:
    • Driver’s License of all tenants on the Lease
    • Income Verification or Tax Returns
    • Signed Lease Agreement (all pages)
    • Rental Ledger March 2020 (if applicable) to current
    • Detainer Warrant or Attorney Letter (if applicable)
    • GNF Disclosure Forms - Download
    • Determination of Eligibility Form - Download


  • Eligibility & Financial Impact Requirements:
    At least one household member must have either:
    • Received unemployment benefits
    • Had reduced household income
    • Faced major expenses
    • Experienced financial difficulties related to COVID-19 (directly or indirectly)



We must receive ALL requested information, prior to processing your application for assistance.  Program is based upon available funds and tenant eligibility. We reserve the right to terminate the program without written notice.
